1) Yes they are successful in making the foreground, middle, and background. The rocks in the path where he is running is the foreground. The lava part would be the middle ground. The background is the bigger rocks.
2) They used three different colors for the rock and lava. They used the dithering technique to show the 3D effect of the rocks in the front, and the character itself. Its applying shape, texture, and value to the graphic.
3) This image has a line element to it. The rock has different lines going all around it. The ceiling above him is fairly simple with lines. There is a lot of different colors, red, yellow, brown(different shades of brown), and different shades of orange.
